Getting Started is Simple
1. Sign Up
Enroll 12 months pre-release or within 2 years of recency. We will add you to our Electronic Health Record and prepare for your release.
2. Release
Within 72 hours or release, a Patient Sortal team member will contact you to schedule your intake and gather consents.
3. Intake
An intake specialist will see you within 7 days of release to verify insurance, assign a care manager, and assess your immediate needs
4. New Patient Appointment
You'll see your Primary Care provider within 14 days of release for a New Patient Appointment, Rx Refills, referrals, and next steps.
