Our Community-based and Patient-Centered approach allow our care managers to help throughout community reintegration. Our services begin 12 months pre-release and we are here to help every step of the way.
Reentry and community partners allow us to ensure our members have housing, education, employment, and workforce development opportunities available. Together we will decrease recidivism and enhance community reintegration.
